Important RMA Procedures

1. Call 800-706-2284 or email your ASI representative for an RMA number.
2. Please fill out the form below to submit your RMA online.
3. Please provide as much detail as possible about the nature of the problem, including the circumstances under which the failure has occurred.
4. Forms required for shipment back to ASI are listed below. These must be completed before shipment.
5. If you have a return that was purchased from a 3rd party and do not have an ASI account, please register one by filling and emailing this form.

6. After submitting and confirming your RMA form, please send your system along with the forms to:

7. Please write the RMA number on the outside of the shipping box when returning the items.

International Shipping Instructions and Forms

  • Please note ASI will only accept shipments for repairs & returns that follow the below procedures.
  • ASI is not responsible for any custom duties, fees, tariffs, merchandise processing, and/or storage when shipping a RMA back to the US for repair. This is the responsibility of the customer.
  • If ASI is billed for custom duties because the following procedures are not followed ASI will bill the shipper for these charges.
  • ASI recommends using FedEx as a shipping courier.
  • Please email your completed Declaration for Free Entry of Returned American Products and Foreign Shippers Declaration of US Goods Returned forms and other relevant paperwork that you are sending with the return shipment, to nicole@asiimaging.com, your ASI representative.
  • Please place a copy of all forms in and write the RMA # on the outside of the shipping box.
  • Remember to include your EORI number if you reside in European Union.
  • On each form clearly state: “This is US goods returning to manufacturer for repair and then to be returned. “
  • Shipment Purpose: Return and Repair
  • We recommend that you declare a low value. This is not a new purchase and this shipment may be subject to custom fees. When we return the repair, we declare the repair value.
  • Use the harmonized code: 9801.00.1012. This harmonized code is for equipment repairs and returns. “Articles returned temporarily for repair, alteration, processing or the like, then foregoing to be re-exported”
  • When we finish the repair, we will declare the amount of repair with the associated commodity HTS.
  • Please keep your export records noting your repair shipment. You will need this documentation to re-import your repair and avoid duties.
